Summary Care Record

The Summary Care Record (SCR) is being introduced for patients across England to improve the safety and quality of patient care. The SCR contains details of medication, allergies and adverse reactions from the patient’s GP record. Additional information may be added from the GP record at the request of the patient; this may include significant past and current medical history, care plans, chronic disease management, palliative care needs, and patient wishes or preferences including Advanced Directives (‘Living Wills’). Any additional information over and above medication, allergies and adverse reactions should only be included with a patient’s explicit consent.
